This is speculation only, but I've got several interesting ideas.
4) You go into the older memory chamber before getting a spirit guide. You do the whole silver/amber thing, but when you go to Dream, you get crushed.
5)after Achenar cracks open the Beauty and the Beast-esque dome (over a flower, right shape...), you are stupid enough to open the door, and you suffocate.
6) umm...the camodile eats the throwing-mangeree? lol
